League Manager's Responsibility Form
I,   (manager's name) acknowledge that I am the manager of   (team name)  which plays at   (Sportsplex name) on   night.   I agree that I am, among other things responsible for the following:
  1. Payment of all league and tournament fees owed to Wessel Sports for the 2008 season.
  2. To keep control and discipline my team both on and off the field to the best of my ability.
  3. Make sure that all players are waivered and rostered on my team.
  4. Notify the park office of all forfeits in advance when possible and to keep in communication with the park manager regarding all schedule changes.
  5. I also acknowledge that I have received and understand the ball policy of Wessel Sports.  (Please review the ball policy for each park by choosing the link for the park you are participating at during the 2008 season.)
Failure to comply with any of the above items may result in suspension of your team and yourself from all play at Wessel Sports facilities and could result in legal action.
